Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Sri Lanka, and contain at least two blank visa page
• Fully completed and signed Application form
• Passport-type photographs: 2
• Passport must be signed
• Must include name and title of person replacing for PCS
• PCS must complete additional visa assignment form
• Visa valid 6 mos.- M/E
• Completed Sri Lanka Traveler Information form
• Complete the Entry Visa Application Form and email it your POC along with a copy of your bio page to obtain the DIP Note.
