Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Cambodia, and contain at least two (2) blank visa pages

• 2 Fully completed and signed Application form 
  • Original wet signature – per traveler

• Passport-type photographs: 2 passport sized (Taken in the last 6 mos.) PLEASE DO NOT STAPLE THE PHOTO – per traveler

• 2 Color copy of Bio page  – per traveler

• Copy of PASSPORT BIO PAGE AND VISA PAGE OF THE SPONSOR- (for dependent & Spouse)

• MARRIAGE CERTIFICATE for Spouse

• BIRTH CERTIFICATE for minor dependents

• 2 COPIES Need Agency Letter 

• 2 COPIES STATE MEMO – we (CVPS) will provide

Note – Letters must have specific information of when, where and what is the traveling going to be doing. It also needs to include the name of the person and the section of the Embassy that the traveler is meeting with.

• For Assignment, provide person replacing (incumbent’s) information, Full name and Title.

Visa is valid 90 days M/E – Issued by Embassy

Minimum Processing Time –   2 – 4 weeks min
