• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity and contain 2 blank pages

• 2 typed and signed visa application Form

• Passport-type photographs: 2 (taken in the last six months)

• State department letter

• Colored copy of Passport Data Page

  • Objects to Israel. (no Israel visa/stamps accepted)

Permanent Change of Station (PCS)

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity and contain 2 blank pages

• 2 typed and signed visa application Form

• Passport-type photographs: 2 (taken in the last six months)

• State department letter

• Colored copy of Passport Data Page
