Visa Requirements

• Original, signed United States Diplomatic or Official passport with at least 6 months of remaining validity, after completion of stay in Bolivia, and contain at least two blank visa page
• 1 Fully typed, completed and signed Application form(must be typed online)

• Submit travel Itinerary
• Applicants must contact the appropriate section/agency at US Embassy La Paz prior to visa submission in the event notification must be made to the Bolivian MFA. This procedure must be followed by all applicants (TDY & PCS) or the visa may not be issued.
• Passport-type photographs: 1 glued
• Visa valid 30 days M/E
