Procedures for Submitting Diplomatic Passport Applications

CVPS Order Form

US Direct Hire: Submit items 1 through 4

  1. Completed passport application(s):

Applications by Mail (DS-82 Form) need to be signed and dated.

However, customers using DS-11 (First-Time and Minors)

apply in person and must not sign the form until in the presence of a

passport office or acceptance facility agent.

  1. Two identical passport photos. Available at the Classified VPS Office, Room C1-7, RRB, Phone 202.842.8617; Fax 2.2.842.8619. If you provide your own photos, it must have been taken within the last 6 months, in normal attire and must be 2”x2” with a white/off-white background, no glasses, with neutral facial expression without showing of any teeth.

  1. Proof of Citizenship.

A previously issued U.S. Passport (valid or expired);

US Personal Services Contractor (PSC’S):

(8) A copy of Contract pages showing signatures and expiration date- Contract

Completed Diplomatic Passport Information Sheet – Sponsor’s information

Copy of Travel Authorization listing your dependent/s as authorized to travel or OF 126 – Foreign Residency

and Dependency Report signed by an approved AID/OHR (newborn dependent only)

Email to us the above completed forms, so that we may provide the Agency Memo authorizing the diplomatic passport request.

 Diplomatic passport requirements for minors –

  • Original Birth Certificate or Certified Copy
  • Completed but not signed DS 11

  • Copy of OF-126 or Travel authorization listing the dependent/s

Passport applications for children under age 16, who have or have not had a passport in

the past, must be submitted in person at a passport agency or authorized passport

application acceptance facility. The child must be present. Both parents and guardians should appear with the child.

 They must go through a special process to confirm that your minor dependent/s may go through the process of applying for the Diplomatic passport.

If you’re going to the US Post Office, you need to instruct the Post Office Passport Acceptance

Officer to seal the application/s in an envelope, enclose a copy of the Agency Memo and to give it back to you for Agency processing (“Hand Carry”).

Attach on the outside of the sealed envelope a copy of the Agency Memo.

DEPENDENT – First Time Passport

For Diplomatic passport, you need to submit –

Completed Classified VPS Order Form

Completed Diplomatic Passport Information Sheet – Sponsor’s information

Copy of Travel Authorization listing your dependent/s as authorized to travel 

Email to us the above completed forms, so that we may provide the Agency Memo authorizing the

diplomatic passport request.

 

Diplomatic passport requirements for Naturalized citizen –

  • Signed Agency Memo (we provide based on the information provided on the Diplomatic Passport Information Sheet)
  • Travel authorization listing the dependent/s

An acceptance officer would be involved since your spouse is applying for a passport for the first time.

The process would have to be facilitated ‘in-person’ meaning that your spouse would have to go to the Passport Office or to a US post office so that he/she could go through a special process to confirm that he/she may go through the process of applying for the passport.

If you’re going to the US Post Office, you need to instruct the Post Office Passport Acceptance Officer to seal the application/s in an envelope, enclose a copy of the Agency Memo and to give it back to you for Agency processing (“Hand Carry”).

For Diplomatic passport, you need to submit the following:

Current valid US Passport – since you are overseas, you may present your current US Passport at the Consular Section of the US Embassy they will make a colored copy and certify/stamp the copy. Special Issuance Agency will accept it in lieu of your passport.

Completed and signed DS 82 Completed traveler information Travel Authorization

2 passport photo- it must be taken within the last 6 month, in normal attire, and must be 2”x2” with a white/off- white background, no glasses, with a neutral facial expression without showing of any teeth.

Bring the printed online DS 82 with barcode and original signature along with our Order form to:

For a second diplomatic passport*, the following must be submitted:

  • DS-82
  • Current passport photo
  • Most recent passport*
  • Memo provided justification for a second passport
  • Diplomatic information sheet

Plus, if USPSC –

  • Contract Cover Page
  • Contract Page Listing Renewal Periods

*If you are requesting the renewal of your 2nd Diplomatic passport, you also need to submit your primary Diplomatic passport

For lost or stolen passport –

  1. Original Birth Certificate /Proof of Identification and Citizenship
  2. Completed but not signed DS 11
  3. 2 Passport photos
  4. DS 64 – Lost or Stolen passport form
  5. Letter of Request – We prepare it with your information (diplomatic passport information sheet)
  6. Copy of Travel Authorization and sponsor’s Diplomatic passport bio page
