First time adult applicant (ages 16 and older)

You Must Apply in Person If:

  •  You are applying for your First U.S. passport
  •  You are under age 16
  •  Your previous U.S. passport was issued when you were under the age of 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago
  • Your name has changed since your U.S. passport was issued and you are unable to legally document your name change

  • Submission of application for an adult in person:

Fill out Form DS-11 (Application for a U.S. passport) Click Here Form DS-11 may be filled out online or by hand, but please note the following:

  1. Must not sign the application until instructed to do so by the Acceptance Agent
  2. Must provide your SSN in accordance with 26 U.S.C. 6039E.  Failure to provide information requested on the form, including your social security number, may result in significant processing delays and/or the denial of your application.
  •  Submit Completed Form DS-11 in person:
    1. You must submit Form DS-11 in person at an Acceptance Facility or Passport Agency
  • Submit Evidence of U.S. Citizenship

When applying for a U.S. passport in person, evidence of U.S. citizenship must be submitted with Form DS-11 Such Evidence can be:

    1. Previously issued, undamaged U.S. Passport
    2. Certified birth certificate issued by the city, country or state
    3. Consular report of birth abroad or Certification of Birth
    4. Naturalization Certificate
    5. Certificate of Citizenship
  • Present identification

When applying for a U.S. passport in person, acceptable identification must be presented at the time of application. Such as:

      1. Previously issued, undamaged  U.S. passport
      2. Naturalization Certificate
      3. Valid Driver’s License
      4. Current Government ID (city, state or federal)
      5. Current Military ID (military and dependents)
  • Submit photocopy of the identification document(s) Presented in step 4
  • Pay the applicable fee which can be found at State department  Click Here
  • Provide One Passport Photo

 

To avoid processing delays, be sure your photo meets all the requirements and are properly submitted with your application.

U.S. Passports for Minors (under age 16)

U.S. Passport for minors (15 and under)

  • Both parents must be present to apply for the minor’s passport (Otherwise you must fill out a DS-3053  Statement of consent or special circumstances: Issuance of a passport to a minor under age 16)
  • Minor has never lost a U.S. Passport
  • Minor must be a U.S. born citizen, naturalized citizen of the U.S. or born abroad to parents of  U.S. citizenship

Submission of application for minor age 15 and under:

  1. Complete Form DS-11: Application For A U.S. Passport
  2. Submit Form DS-11: Application For A U.S. Passport In Person
  3. Submit Child’s Evidence of U.S. Citizenship (submit one of the following).
    1. Previously issued, undamaged U.S. Passport
    2. Certified Birth Certificate issued by the city, country or state*
    3. Consular Report of Birth Abroad or Certification of Birth
    4. Naturalization Certificate
    5. Certificate of Citizenship
  4. Submit Evidence of Relationship Between Child and Parent(s)/Guardian(s)
    1. Minor’s certified U.S. birth certificate with both parent’s names
    2. Minor’s certified Foreign Birth Certificate with both parent’s names*
    3. Minor’s Report of Birth Abroad with both parent’s names*
    4. Adoption Decree with adopting parent’s names*
    5. Court Order establishing custody
    6. Court Order establishing guardianship
  5. Present Identification of Parent(s)/Guardian(s) (one of the following)
    1. Naturalization Certificate
    2. Valid Driver’s License
    3. Current Government Employee ID (city, state or federal)
    4. Current Military ID (military and depentents)
  6. Submit a Photocopy of Each Parent/Guardian ID Document To Be Presented (Step 5)
  7.  Provide Parental Consent Form DS-3053
  8. Pay the State Department Applicable Fee ; plus our CVPS service fee
  9. Submit Two Passport Photo for photo requirements. To avoid processing delays, be sure your photo meets all the requirements and are properly submitted with your application.
  10.  CVPS Order Form with original signature otherwise application will not be accepted for processing

If you wish to mail in your application to Classified VPS please make sure you have included all required documents outlined in steps 1-10.

Note: Classified VPS strongly suggests using a traceable delivery method such as FedEx. In order to protect the contents of your mailing from the elements throughout the delivery process, it is strongly recommended that you mail your passport application and personal documents in an envelope large enough to fit the application without folding. Please do not use a standard letter-size #10 (4 1/8″ x 9 1/2″) envelope.
